We know that many specialty therapies require additional monitoring to help a patient remain successful on their provider’s recommended treatment plan. Therefore, instead of waiting for a patient to reach out to us, our CVS Specialty CareTeam (with the help of our proprietary data engine), establishes a conversation with the patient so we can actively monitor their progress and help them have a voice in their care. We assess medication effectiveness, help resolve symptom experiences, manage adherence and calculate medication accumulation – intervening when necessary to improve outcomes.
